This badge recognizes exceptional performance in key areas such as clinical quality, operational performance, and overall patient satisfaction. Some quality care measures that Frontera excelled in this year to earn this recognition include their work in hypertension management, diabetes control, and depression screenings and follow up. This is the first time in Frontera’s 17 years of existence that they have been honored with this award for two consecutive years in a row. There are approximately 1,400 federally qualified health centers nationwide. Frontera was one of 160 health centers to earn a Silver Badge, with only 12 of those coming from Texas. This badge demonstrates Frontera’s dedication to living out their mission by continuously improving the quality of care they provide to their patients.

This badge is awarded to health centers that meet the following criteria:Adopted an electronic health record (EHR) system.
Offered telehealth services.
Exchanged clinical information online with key providers health care settings.
Engaged patients through health IT.
Collected data on patient social risk factors.
Advocacy Center of Excellence - Gold
This prestigious recognition is a testament to Frontera's dedication and prioritization of advocacy. An Advocacy Center of Excellence (ACE) is a Community Health Center that creates a culture of advocacy to ensure that policymakers at all levels of government commit to investing in the affordable, equitable, and innovative care that health centers provide. ACE levels recognize consistent engagement and demonstrate an ongoing commitment to making advocacy an organizational priority.
Patient-Centered Medical Home
Patient-Centered Medical Home (PCMH) model is an approach to delivering high-quality, cost-effective primary care. Using a patient-centered, culturally appropriate, and team-based approach, the PCMH model coordinates patient care across the health system.
The PCMH model has been associated with effective chronic disease management, increased patient and provider satisfaction, cost savings, improved quality of care, and increased preventive care.
